How does the program work?

Liberty delivers diabetes testing supplies, diabetic products and prescriptions right to your door. In addition to Medicare we now work with many private insurance plans. Our trained representatives fill out your claim forms and bill your insurance providers. We also connect you to pharmacists and diabetes professionals. So help is always available when you need it. How much do I pay for my diabetes testing supplies? How much does Medicare cover?

How much you pay will depend on your insurance coverage and the state in which you live. Medicare generally pays 80% of the cost of diabetes testing supplies and ostomy supplies (100% of the cost of hemoglobin A1c tests) for qualified beneficiaries—after any applicable co-payments and deductibles have been met.

If you have qualified supplemental insurance, you may be covered for the remaining portion—so you pay nothing at all. However, you will be responsible for any portion of diabetes products (co-pay or deductible) that are not covered by your insurance.

If I have Medicare, do I need to have supplemental insurance to enroll with Liberty for diabetes supplies?

No. However, if you have no supplemental insurance, you will be responsible for costs not covered by Medicare. Medicare generally pays only 80% of the cost of diabetes testing supplies for qualified beneficiaries—after any applicable co-payments and deductibles have been met. If you have no supplemental insurance, you will be responsible for the remaining portion (generally 20%). In addition, you will be responsible for co-payments and deductibles not covered by your primary insurance.

Is Medicare the only insurance plan Liberty accepts?

No. In addition to Medicare, Liberty accepts insurance plans. We also accept Medicaid in most states, as well as a variety of prescription drug cards. Call us at 1-800-705-5797 to see if your insurance qualifies.

What Medicare-covered items does Liberty provide?

Liberty provides the following Medicare-covered items to qualified beneficiaries (co-payments and deductibles apply):

What should I do if I can't afford my diabetes testing supplies or prescriptions?

Who'll fill out my claim forms?

As a participating Medicare provider, and provider for most commercial insurance plans, Liberty fills out your claim forms and bills your insurance plans directly.

Who's eligible?

If you are on Medicare, you may be eligible to receive your diabetes or other medical supplies at little or no cost**. Many people with private insurance plans are also eligible, and in certain cases, depending on the medical products you need, it may also be possible to pay cash for your items. Finally, you may qualify to recieve a free glucose meter.
